Information for New Patients
We welcome you to our practice. We know this can be a very anxious time for you and we want to make you feel as comfortable as possible. To provide the best possible care, we need to get to know you.
Before your first appointment in the office (even if you were seen in the hospital) you should receive a packet of forms to complete and bring with you to the appointment. Also please remember to bring your insurance cards.
If you do not receive this packet please call our office at 860-439-1770 and we will be happy to send one out to you right away. The packet includes a form for personal and insurance information, a letter which must be signed and returned explaining what is required from the patient regarding billing, an authorization for assignment of benefits and authorization to release information, along with our privacy practices. There is also an extensive health assessment form that we ask you spend time filling out prior to your visit so the physician can review it with you at the time of your appointment.
Please make arrangements for your medical records to be sent to our office. Your records should be received at least two days prior to your appointment so the physician can review them.
Any insurance referrals required must be obtained PRIOR to your visit. Please make sure your referral has been approved so there will not be a problem on the day of your appointment. We participate with most health plans but as there are so many different types, you must be sure of your individual coverage and referral prior to your visit.
